Thank you for scheduling your appointment at Los Olivos Women’s Medical Group. For information about the group, please see the Los Olivos brochure. You will be scheduled for your appointment and given access to the patient portal. Please complete the link sent to your text within 90 minutes of receiving. it. You can also request and appointment on the Stanford website.
Los Olivos Women's Medical Group has partnered with Stanford University HealthCare alliance. UHA is a network of board certified primary care and specialty physicians loacated throughout the Bay Area. Los Olivos physicians admit patients to Good Samaritan Hospital in San Jose for inpatient Obstetric and Gynecologic care.
On the day of your appointment, please bring a photo identification and your insurance card. Please complete the New Patient Information.
Please schedule an appointment at Los Olivos by calling or emailing the receptionist of the physician that you would like to see. Most physicians will schedule you for a pregnancy confirmation visit that includes an ultrasound for your first visit. You will receive a copy of the Los Olivos Pregnancy Book at your first obstetric appointment. Please complete the Genetics Questionnaire and bring it to your first appointment. If you have not been to Los Olivos in the past, please also complete the New Patient Information.
If you would like to allow Los Olivos to discuss your medical care with a family member or other designated individual, please also complete the Information Authorization form.
If you are under 18, please also complete the Minor Consent for treatment.
